SMART on FHIR 應用程式市集與國際實務比較：兼論醫療科技 AI、預防醫學與自費健檢之深度分析(2)
第二章：臺灣推動智慧醫療與資料標準化之戰略佈局與徵案分析
臺灣在推動數位醫療與智慧照護方面，擁有得天獨厚的雙重優勢：一方面是全球推崇、涵蓋率極高的全民健康保險（National Health Insurance, NHI）制度，累積了數十年的縱向人口健康數據；另一方面則是具備強大硬體製造與軟體研發實力的資通訊（ICT）產業 。然而，儘管具備這些優勢，臺灣的醫療體系在實務上仍長期受困於各家醫院各自為政的醫療資訊系統（HIS）。不同醫療機構所採用的資料庫架構、編碼系統與傳輸協定互不相通，導致醫療數據嚴重碎片化，成為發展跨院區智慧醫療、精準醫療與連續性照護的重大阻礙 。
為了從根本上解決此一結構性問題，衛生福利部（MOHW）資訊處近期正式啟動了「臺灣醫療資訊標準大平臺」（Taiwan Medical Information Standards Platform），提出了一套宏大的國家級數位轉型戰略。該戰略以「數據（Data）」、「規則（Rules）」與「應用（Applications）」三大支柱為核心架構，力圖在兩年內將全國 23 家醫學中心的電子病歷資料標準化並推升至雲端環境，最終實現病患跨院就診時免除申請實體病歷的無縫接軌願景 。
數據與規則的底層重構：FHIR 與 CQL 的協同作用
在「數據」層面，衛生福利部採取了由上而下（Top-Down）的強勢推導策略，全面導入國際醫療資料標準 FHIR 作為新一代電子病歷交換的共通語彙 。此舉強制規範了醫療數據在儲存與傳輸時的格式化標準，確保跨機構間的資料不僅在語法上互通，在語意上亦能精準對齊，徹底消除過去因系統異質性所導致的資料轉譯誤差。
在「規則」層面，衛福部大膽引入了臨床品質語言（Clinical Quality Language, CQL）技術，旨在建構一個全國統一且機器可讀的「規則庫（Rule Library）」。CQL 是一種專為臨床決策支援與品質測量所設計的標準化表達語言。透過 CQL，複雜的健保申報規範、實證醫學的臨床治療指引（Clinical Practice Guidelines）以及醫療品質評估指標，皆可被轉化為可執行的電腦邏輯。當此規則庫與 FHIR 標準化數據相結合時，醫療機構便能建構出強大的智慧臨床決策支援系統（Clinical Decision Support Systems, CDSS）。這使得醫師在開立處方或進行診斷時，系統能即時自動檢核其決策是否符合最新醫療標準與健保規範，從而大幅減少人為醫療疏失，並優化健保行政審查的效率 。
「臺灣50」優良 SMART on FHIR 應用程式徵案活動
在確立了堅實的數據與規則底層基礎後，衛生福利部將戰略目光轉向了「應用」層面，正式宣告比照 Google Play Store 或 Apple App Store 的商業模式，打造專屬於醫療領域的「AI 醫療應用生態系統」。為充實此生態系的內容並樹立標準典範，衛福部發起了「臺灣50」優良 SMART on FHIR 應用程式徵案活動 。
本次徵案活動的報名期間設定為 2025 年 11 月 1 日起至 12 月 31 日 17:00 止，目標在於遴選出首波 50 款具備高度臨床實用價值且完全符合國際標準的優良應用程式 。主辦單位針對技術合規性提出了嚴格要求，所有參賽作品必須嚴格遵循 SMART on FHIR 標準的 API 介接與授權規範，並且必須在衛福部資訊處所建置的「測驗沙盒環境（Sandbox）」中成功完成上架驗測程序 。這套沙盒環境模擬了真實的醫療數據與 OAuth 2.0 授權流程，確保了每一款入選的應用程式在未來實際導入醫院時，皆具備隨插即用的穩定性與安全性。
本次徵案涵蓋了四大核心優先領域：「醫學 AI 類」、「營運管理類」、「臨床決策支援類」與「資料視覺化類」。這樣的分類規劃顯示了政策的全面性，不僅關注前瞻性的人工智慧診斷與臨床決策輔助，亦重視醫院內部的行政營運效能提升，以及龐大醫療數據的直觀呈現與解讀。獲選的 50 款優良應用程式將獲得政府的獎勵與官方曝光，並正式上架至「臺灣醫療資訊標準大平臺」轄下的 TW APP Gallery 專區 。此專區將成為全國各級醫院與醫療機構在尋求系統升級或導入數位健康解決方案時的首選官方市集，極大地縮短了醫療機構尋找、評估與採購軟體的時間與成本 。
